Fees
and Payment Policy
In an effort to keep dental costs down while maintaining a high level of
professional care, we have established the following payment plans for
the use of our patients. Fees may be paid as follows:
- Prepayment, or payment at time of
treatment with a 5% discount.
- Payment of balance in full within 10
day of statement due.
- Use of your Visa, Discover, MasterCard
or American Express.
- Extended payment plans available for
qualified patients.
All major treatment and treatment involving a laboratory procedure will
require an appropriate down payment.
Our senior citizens, 65 and over, will receive a 10% discount for
payment at time of treatment.
Insurance
If you have insurance we will help you to determine that coverage you have
available. We ask that you assign your insurance benefit to us. The
balance will be arranged for you to pay as listed above.
Professional care is provided to you, our patient, and not to an insurance
company. Thus, the insurance company is responsible to the patient
and the patient is responsible to the doctor. We will help in every
way we can in filing your claim and handling insurance questions from our
office on your benefit. |
