We know you have many choices when selecting your dentist, and we would
be thrilled to have you as a new patient!
When you come to our
office for your first visit, you will need to bring the following:
- Patient registration forms. You may download and
print these two forms on your computer. If you are unable to
print these forms we can fax them to you or you can come in 15
minutes before your appointment to complete the forms.

Medical
History Form
- Your Insurance card or cards.
